Employee performance is a critical factor in supporting the success of health services in government agencies and is influenced by various organizational factors, such as leadership, communication, and compensation. Differences in employee perceptions regarding these three aspects have the potential to affect the resulting performance levels. This study aims to analyze the influence of leadership, communication, and compensation on employee performance at the Palu City Health Office. This research utilized a cross-sectional design with a quantitative approach. The population and sample for this study consisted of all 111 employees at the Palu City Health Office, selected using a total sampling technique. Data were collected through questionnaires, and data analysis was performed using multiple linear regression. The results indicated that leadership had a significant influence on employee performance with a $p$-value $= 0.000$ ($p < 0.05$), though the direction of the influence was negative. In contrast, communication had a significant positive influence on employee performance with a $p$-value $= 0.000$ ($p < 0.05$), and compensation also showed a significant positive influence on employee performance with a $p$-value $= 0.000$ ($p < 0.05$). Simultaneously, leadership, communication, and compensation significantly influenced employee performance with a $p$-value $= 0.000$ ($p < 0.05$). These findings suggest that communication and compensation are factors that play a positive role in enhancing employee performance, while leadership showed a significant but negative influence at the Palu City Health Office. Therefore, the agency is advised to improve leadership consistency, strengthen communication, and review compensation systems to support the enhancement of employee performance.
